I hereby apply to the City of Monticello for disconnection of secondary water services to my premises at:
I hereby understand the following:
1. Prior to disconnection, I will provide proof of my ownership of the property to which secondary water services are proposed to be disconnected.
2. If, in the future, I wish to reconnect to Monticello City's secondary water system I will pay the necessary reconnection fee as established by ordinance or resolution.
3. I will follow the laws and procedures set forth by the City of Monticello with regard to use of secondary water services.
