BOND REQUIREMENT FOR EXCAVATION OR CUTTING OF ASPHALT
Applicant shall execute a surety bond made payable to the City of Monticello in the minimum amount of $200.00 with an expiration date of no less than one (1) year after completion date

AGREEMENT:
Applicant agrees to notify Public Works Director prior to any commencement of work, and to comply with all conditions of Monticello City Code 7-3 (Attached at top of page)
IT IS UNDERSTOOD THAT:
1. All applications must be FILLLED OUT COMPLETELY AND SIGNED BY MONTICELLO CITY PUBLIC WORKS DIRECTOR, PRIOR TO COMMENCEMNET OF ANY WORK.
3. For one (1) calendar year from date of completion, the contractor is responsible for any needed street patch repairs.
2.The contractor is responsible for backfilling all excavations according to APWA, Asphalt T Patch Specs. Contractor is also responsible for installing compacted asphalt to match existing asphalt thickness plus 1". Non-shrink flowable concrete can be used as a replacement for gravel or road base or be used as surface patch flush with asphalt until permanent asphalt is replaced.
4. Right-of-way must be cleaned of all byproducts of street cut and must be completed before utilities are turned on, unless otherwise noted by the Public Works Director.
5.FAILURE TO COMPLY WITH CITY REGULATIONS, SPECIFICATIONS, OR INSTRUCTIONS PERTINENT TO THIS PERMIT WILL RESULT IN SUSPENSION OF ALL OR PART OF EXCAVATION WORK WHICH MAY NOT BE RESUMED UNTIL SATISFACTORY CORRECTIVE ACTON HAS BEEN TAKEN.
6.No excavation may be performed on Monticello City right-of-way beyond the area stipulated on this permit.
7. Contractor will provide detailed information of work completed including pictures and GPS Data.
